Executive summary

Get there when you’re needed, 2.5× faster.

VitaCare Living of Proctor installed Teton at the end of 2025. Following a January 2026 baseline and a February-to-March onboarding period, we evaluated active use across Q2 2026 against that baseline. Every metric here comes from operational data — not estimates, not extrapolation.

Continuous visibility cut fall rates by 44%, put a staff member at every fall recovery, and turned response into substantive care: caregivers reached residents faster and stayed longer once there.

VitaCare was already a standout — its baseline response times and fall rates sat well ahead of industry norms before the pilot began. The real test was whether Teton could improve outcomes at a community already performing at a high level. It did.

Staff-assisted recovery

100%

Staff present at every fall

Up from 50% self-recovered at baseline.

Starting from strength. VitaCare’s baseline response times and fall rates already sat well ahead of industry norms. The real test was whether Teton could move outcomes at a community already performing at a high level — and it did.

Part 02 · What changed on the floor

Three mechanisms drove the change on the floor.

Continuous visibility changed the floor in three ways: caregivers reached residents faster, spent longer with them once there, and were present for every recovery.

01

Alerts arrive in seconds, not minutes.

Continuous monitoring replaced reactive discovery. Caregivers reached residents in an average of 2 minutes, down from 5 at baseline, and 88% of falls were attended within five minutes, up from 67%.

02

Time with each resident grew.

When caregivers did go in, they stayed longer. Average visit duration rose from 1.7 to 2.2 minutes. Visits became substantive care rather than quick check-ins.

03

Staff present at every fall recovery.

At baseline, half of falls were self-recovered — residents getting up unseen and at risk of a second fall. During the pilot, a staff member was present at 100% of fall recoveries, with fewer secondary falls and clearer cause.

Nighttime visibility · Observation without intrusion

Residents slept longer. Staff knew more.

+17%

more rest per resident each night — about 2 more hours in bed (10.2 → 11.9 hours)

100%

night-shift coverage, with no gaps across nights, weekends or shift changes

Part 03 · The economics

Falls prevented are residents who stay.

The ~30 falls prevented annually translate to meaningful revenue retention. Fall prevention is one of the few operational levers that compounds: each avoided fall protects length of stay, occupancy, and the family’s confidence in the community.

Revenue retained · annual

$45,000

per year at this 30-bed community — a conservative estimate from avoided premature move-outs following fall-related decline. Actual figures typically run higher at market rates.

How we get there

Annual falls prevented at this facility~30
Share that would have led to premature move-out5%
Move-outs avoided per year~1.5
Length-of-stay preserved per avoided move-out~6 months
Monthly rate (conservative)$5,000
Annual retained$45,000

Assumptions based on Teton analysis of similar communities. The calculation uses a conservative monthly rate; actual revenue retention at market rates would be higher.

Part 04 · Adoption & the result

Teton became part of the floor’s daily rhythm.

Adoption was the precondition for every outcome above. Across three months, staff used Teton continuously — not as an add-on, but as the active tool of record for the floor.

What changed in practice

  • Immediate incident awareness, replacing reactive discovery.
  • Faster injury assessment and response, with contextual data attached.
  • Documentation quality lifts as events are observed, not reconstructed.
  • Longer, more substantive visits returned time and trust to the team.
